2. What these terms cover
These terms govern your use of this website. InnovaRx and InnovaMed are separate applications with their own terms, presented at sign-up. Coding, audit and education engagements are governed by the individual agreement signed for that engagement, which takes precedence over anything here.
3. Informational content
Everything on this site is general information about our services. It is not coding advice, legal advice, or a compliance opinion for any specific chart, claim, or organization. Coding decisions remain the responsibility of the coder and the organization submitting the claim.
4. No patient information
Do not submit protected health information through this website. The contact form is for business inquiries and is not a secure channel. See the Privacy Policy.
5. Acceptable use
You agree not to:
- Use this site to break the law or infringe anyone's rights
- Attempt to gain unauthorized access to any system or account
- Scrape, copy, or republish site content without permission
- Interfere with the site's operation or availability
6. Intellectual property
Site content, branding, and the Innova name are ours. ICD-10-CM and the CMS-HCC risk adjustment models are published by CMS and NCHS and are in the public domain. Third-party code sets remain the property of their owners, and are used only where properly licensed.
7. Third-party links
This site links to our own applications and occasionally to external resources. We are not responsible for the content or practices of sites we do not operate.
8. Subscriptions, cancellation and refunds
Subscriptions are bought and managed inside the applications themselves, and the terms presented at checkout govern them. What follows is a plain summary of that policy, not a replacement for it.
Paid plans are billed through Stripe at the price shown at checkout and renew automatically until you cancel. You can cancel at any time from your own account page — no email or phone call required. Cancellation takes effect at the end of the period you have already paid for, and you keep full access until then.
Fees already paid are not refunded, including for the remainder of a period after you cancel. Prices can change, with notice before your next renewal. Free tiers are metered and their limits can change.
Coding, audit and education engagements are not subscriptions. They are governed by the individual agreement signed for that engagement, which sets its own payment and termination terms.
9. Disclaimer and limitation of liability
This site and our software are provided “as is”, without warranties of any kind. We work to keep reference data current with each CMS, NCHS and FDA release, but we do not guarantee that it is accurate, complete, or available at any given moment.
Nothing we publish is a coding determination. Risk scores, code mappings, and medication-to-condition relationships are reference support. Coding decisions belong to a credentialed coder working from the full record and current official guidance, which control over anything shown here. We do not warrant any particular payment, audit, reimbursement, or compliance outcome, and nothing on this site should be read as promising one.
To the fullest extent permitted by law, Innova Health Strategies is not liable for indirect, incidental, special or consequential damages, or for lost profits or lost data. Our total liability for any claim is limited to the amount you paid us for the service giving rise to that claim in the twelve months before it arose.
Nothing here limits liability that cannot be limited by law. Some jurisdictions do not allow certain exclusions, so parts of this section may not apply to you.
10. Governing law
These terms are governed by the laws of the State of Florida.
11. Changes
We may update these terms. The date at the top reflects the current version, and material changes will be identified rather than quietly made.
